At Vitality Wellness we want you to experience healthcare the way it was meant to be accessible, affordable, and centered around you. For a monthly membership fee, patients receive unlimited access to their primary care provider, including same-day or next-day appointments, extended consultations, and direct communication via phone, text, or email. 

• One-time initial consultation: $100
• We do not accept insurance. 
• We charge a monthly membership fee 
  • Individual Adult (18- 65 y/o) $89/month, or $129/month, or $169/month 
  • Additional adult (18-65 y/o) $79/month (one time $100 initial consultation fee)
  • Additional child (2-17 y/o) $39/month (parent must be a member)
• Your insurance can be used for labs, studies and referrals
